- Topic-specific paired cases: 7 male/female pairs (14 synthetic profiles), with worked applications, options and source maps.
- Shared anesthesia options and cases: the same four-modality guide and 12 general teaching cases used throughout the clinical portfolio.
- Example format and verification standard, topic source audit, and shared source audit.

Perioperative Analgesic Strategy in Breast Reconstruction Surgery: A Narrative Literature Review with Evidence-Gap Analysis for Breast Implant Revision
Status Project Type Version Last Updated License: CC BY 4.0
Educational work by a pre-medical student — not medical advice. Not a clinical protocol, prescription, or patient handout, and not for patient care. See DISCLAIMER.
Review status
This remains a narrative literature-review draft. This revision checks framing and selected safety issues; it does not independently certify every citation, extraction, or statement of reference verification. Implant revision, primary oncologic surgery, augmentation, and autologous reconstruction remain distinct evidence populations. See PUBLICATION_REVIEW.md.
Overview
This repository contains a narrative literature review of perioperative analgesia in breast reconstruction surgery, with an evidence-gap analysis for breast implant revision, plus three companion narrative reviews that examine the same setting from distinct analytical perspectives: analgesic strategy, patient communication, and recovery measurement.
All documents are narrative literature reviews prepared as part of an independent academic portfolio project. The main manuscript documents its PubMed search strategy, but it is not a systematic review or meta-analysis. None of the documents constitute clinical protocols, institutional guidelines, order sets, or decision-support tools.
Repository Contents
| File | Description |
|---|---|
| George-BreastRecon-Review-v8.md | Main manuscript (manuscript v8, repository release 2026.11; Markdown only, no PDF) — narrative literature review across five perioperative decision domains, with a documented PubMed search, evidence tables, and an evidence-gap analysis for implant revision |
| 02_Comparative_Educational_Review.md | Narrative review of multimodal analgesic approaches in breast reconstruction revision, including regional anesthesia, opioid-sparing strategies, and ERAS-aligned frameworks |
| 03_Patient_Communication_Analysis.md | Narrative review of patient communication challenges in perioperative counseling, with attention to health literacy, shared decision-making, and informed preference formation |
| 04_Postoperative_Recovery_Considerations.md | Narrative review of postoperative recovery as a multidimensional outcome domain, examining the gap between standard recovery metrics and patient-experienced recovery |
| 06_References.md | Master numbered reference list for the manuscript and all companion documents, with identity-checked PMIDs and DOIs (metadata only; see REFERENCE_AUDIT.md) |
| HISTORICAL_VERSION_NOTICE.md | How to read earlier versions recoverable from Git history |

File numbering: the companion documents keep their original numbers (02, 03, 04, 06) so that existing links continue to work. Numbers 01 and 05 are not used; the main manuscript takes the place of an overview document.
Intellectual Architecture
The three companion documents examine distinct but related dimensions of the same perioperative setting:
- 02 addresses what the published literature says about analgesic strategy and its measured outcomes
- 03 addresses how communication quality shapes whether patients can meaningfully participate in those decisions
- 04 addresses whether the outcome constructs used to evaluate analgesic strategies adequately capture patient-experienced recovery
The main manuscript brings these together with two further domains (regional techniques and adjuvant pharmacology) and an evidence-gap analysis for implant revision.
What This Repository Is Not
This is not:
- Medical advice or clinical guidance of any kind
- A clinical protocol, institutional guideline, policy, or order set
- Approved or validated for clinical implementation
- A substitute for attending anesthesiologist or surgeon judgment
- A systematic review, meta-analysis, or PRISMA-compliant report
- A preclinical or clinical research package
- A regulatory submission or product development document
- A document reviewed, approved, or authorized by any institution
- An endorsement or representation of the views of any institution, faculty member, or clinical educator
Interpretive Caution
The literature reviewed in these documents spans heterogeneous clinical populations, institutional practices, anesthetic techniques, and evidence quality levels. All documents are narrative literature reviews. No PRISMA framework, formal risk-of-bias assessment, GRADE rating, or data pooling was applied. Evidence strength is described only by study design (for example, "meta-analysis of RCTs").
Findings discussed across these documents reflect the published literature and should not be read as validated clinical guidance, performance targets, or prescriptive standards of any kind.
Intended Use
This repository is appropriate for:
- Scholarly review of perioperative analgesic literature in breast reconstruction revision
- Educational discussion of patient communication and informed consent in perioperative settings
- Academic analysis of recovery outcome measurement
- Independent academic portfolio presentation
It is not intended for patient care, clinical decision-making, institutional implementation, or documentation use.
Evidence Base
The documents draw on published literature including the ERAS Society consensus review for breast reconstruction, the PROSPECT recommendations for oncological breast surgery (2020, historical, and the 2026 update for major surgery), the SAMBA practice advisory on pectoralis blocks, the 2026 ASA practice guideline on local and regional analgesia (including mastectomy), ASRA Pain Medicine guidance on local anesthetic systemic toxicity and antithrombotic therapy (fifth edition, 2025), the international IV-lidocaine consensus statement, the Fifth consensus PONV guidelines (the Fourth is cited as historical), randomized trials and meta-analyses on regional techniques and opioid-sparing adjuncts, health literacy and shared decision-making research, and perioperative recovery measurement research.
Reference identifiers are provided for audit; this release-preparation pass does not establish that every citation and supporting claim has been re-verified. Full references with PMIDs and DOIs are provided in 06_References.md.

Superseded guidance is now labeled as historical in the document bodies, and the current versions are cited and listed in 06_References.md (entries 60–66). These are the 2026 PROSPECT update for major oncological breast surgery, the Fifth PONV consensus, the ASRA antithrombotic guideline fifth edition, the 2026 ASA local and regional analgesia guideline, the Foo et al. 2021 IV-lidocaine consensus and the Verret et al. 2020 gabapentinoid meta-analysis. Citing a newer guideline does not make this portfolio an implementation guide. See RELEASE_NOTES.md for details.
