Clinical example format and verification standard

Educational work by a pre-medical student — not medical advice. Not a clinical protocol, prescription, or patient handout, and not for patient care. See DISCLAIMER. Version: 2026-10-28

This is an author-created educational documentation standard, not a professional practice guideline, certification or validated assessment instrument.

Every topic-specific example pair records:

  1. Entirely synthetic provenance, procedure/clinical question and relevant male/female profile details.
  2. The patient factor that changes the discussion; sex alone is not treated as a dose or treatment rule.
  3. Worked reasoning, conditional options, administration context and decision flow.
  4. Intended effects, adverse effects and reassessment questions.
  5. A claim-to-source map with DOI, source type, population, version and applicability limits.

For any future numerical dose example, record the exact supporting passage, indication, route, preparation, weight metric, units, calculation, bounds, organ-function considerations and monitoring context before inclusion. Separate source dose information from authored arithmetic and model outputs. Do not convert an incomplete fictional profile into a prescription, automatic escalation algorithm or pump setting.

Verification levels are distinct:

  • Identity: title, journal/year and DOI refer to the intended source.
  • Content: abstract or selected source passages support the scoped concept.
  • Applicability: describe differences between the source population and fictional case.
  • Full appraisal: complete full text, supplements, corrections and evidence synthesis; this rollout does not claim this level for every source.
  • Clinical validation: patient outcomes and safety evaluation; none is established by these examples.

A DOI or peer-reviewed venue does not establish that a fictional plan is correct, that a product is clinically validated, or that every recommendation is current or universally applicable. Official society standards and prescribing information may lack DOIs; identify them separately rather than inventing identifiers. Preserve source copyright and do not reproduce proprietary questionnaires, figures or algorithms as author-owned content.

Use source versions and verification notes in EXAMPLE_SOURCE_AUDIT.json and SHARED_SOURCE_AUDIT.json. The shared guide may link to the existing Smart NOA source audit under a different filename. Existing manuscripts/PDFs and historical claims are not retrospectively verified by adding this case set. All real clinical choices remain the responsibility of the treating team within the applicable setting.
