Post-mastectomy breast implant revision — educational dosing reference, November 2026

How to read this comparison

This is a side-by-side summary of Approach 1, Approach 2 and Approach 3. Numeric values are tagged to DOSING_SOURCES.md and are illustrative. No universal first-, second- or third-line order is supported. The numbering is for reference only. Selection is individual: procedure, comorbidities, contraindications and patient preference.

Values changed from v2.0, and v2.0 values removed for lack of an acceptable source, are listed in RELEASE_NOTES.md.

Abbreviations

OFA, opioid-free anesthesia; PECS, pectoral nerve blocks; PONV, postoperative nausea and vomiting; LAST, local anesthetic systemic toxicity; MME, morphine milligram equivalents (oral); OSA, obstructive sleep apnea; TOFR, train-of-four ratio; IBW, ideal body weight.

Questions to ask of any study before applying it

Domain Question
Population Augmentation, oncologic surgery, implant revision or autologous reconstruction?
Intervention Which drugs, blocks, infiltration and rescue were actually used?
Comparator Was background multimodal care comparable?
Outcomes Pain, opioid exposure, nausea, respiratory events, hemodynamics, function?
Harms Were adverse events defined and actively collected?
Applicability Does the evidence apply to implant revision?

Side-by-side features

Feature Approach 1: block + GA Approach 2: GA, no block Approach 3: opioid-inclusive GA
Regional anesthesia PECS I+II or serratus None (surgeon infiltration usual) None (infiltration helpful)
Intra-op opioids None planned; small rescue acceptable None planned; small rescue acceptable Fentanyl ± hydromorphone, individualized S18, S19
Local-anesthetic toxicity concern Yes (block ± infiltration) Yes (infiltration) Yes if infiltration is used
20% lipid emulsion Immediately available S3 Immediately available S3 Immediately available if infiltration S3
IV lidocaine Not used with blocks S1 Not combined with infiltration S1 Not combined with infiltration S1
Main safety concerns LAST; dexmedetomidine bradycardia/hypotension S14; ketamine sympathomimetic effects S15 Same, plus sedative stacking Opioid respiratory depression; PONV
PONV prophylaxis By Apfel factors S9: two interventions for 1–2, three to four for ≥3 S7, S8 Same Same; postoperative opioids add one Apfel factor S9
Reversal Quantitative TOFR ≥0.9 S10; sugammadex by depth S11; neostigmine only at minimal depth S10, S12 Same Same

Removed from v2.0 (no acceptable source):

  • the "First-/Second-/Third-line" labels;
  • "lowest / low / higher PONV" rankings;
  • "very smooth / slower" emergence ratings;
  • "best for" lists that implied superiority;
  • PACU vital-sign frequencies, SpO₂-monitoring durations and RASS intervals per approach.

Adjunct dose summary (label or consensus values)

Agent Value Approaches Source
Dexmedetomidine (off-label as GA adjunct) Label procedural-sedation reference: load 1 mcg/kg over 10 min; maintenance 0.2–1 mcg/kg/h. >65 y: load 0.5 mcg/kg, reduce maintenance. Reduce in hepatic impairment 1, 2; optional 3 S14
Ketamine (subanesthetic; off-label analgesic use) Bolus ≤0.35 mg/kg; infusion up to ~1 mg/kg/h 1, 2; optional 3 S15
IV lidocaine ≤1.5 mg/kg IBW load over ~10 min; ≤1.5 mg/kg/h IBW; ≤120 mg/h; ≤24 h; not under 40 kg; not concurrent with blocks or infiltration, with ~4 h separation Rarely applicable (only with no other local-anesthetic intervention) S1
Propofol Induction ~2–2.5 mg/kg (<55 y) or ~1–1.5 mg/kg (elderly, debilitated, ASA III–IV). Maintenance 100–200 or 50–100 mcg/kg/min All S16
Rocuronium 0.6 mg/kg; RSI 0.6–1.2 mg/kg All S17
Fentanyl Individualized; label low-dose tier 2 mcg/kg; reduce in elderly or debilitated patients 3 (rescue in 1–2) S18
Hydromorphone 0.2–1 mg IV slowly; label interval every 2–3 h; lower start in elderly; ¼–½ in hepatic or renal impairment 3 (rescue in 1–2) S19
Sugammadex 2 mg/kg (T2 reappeared); 4 mg/kg (1–2 post-tetanic counts) All S11
Neostigmine + glycopyrrolate 0.03–0.07 mg/kg (max 0.07 mg/kg or 5 mg) at minimal depth only; glycopyrrolate 0.2 mg per 1 mg neostigmine All S10, S12, S13

What the cited evidence supports (directional)

Outcome Direction Author's certainty judgment
Early pain and 24-h opioid use Lower with PECS/serratus block vs no block (meta-analyses in 06) Moderate
PONV Lower with opioid-sparing techniques and TIVA; reduced by multimodal prophylaxis S7, S8 Moderate
Hypoxemia / bradycardia Higher with dexmedetomidine-based OFA vs opioid-based (POFA RCT) Moderate (single large RCT)
PACU duration, satisfaction, admission No direct comparative data for implant revision Very low / none

Selection considerations (not a decision rule)

Situation Considerations
Block feasible and accepted Approach 1 offers block-related early analgesia; weigh block risks and the shared local-anesthetic budget S4, S5
Needle aversion, block contraindicated or declined Approach 2 or 3. Infiltration quality matters; IV lidocaine is not combined with infiltration S1
Antithrombotic therapy Superficial plane blocks may be acceptable. Decide by agent, dose, timing and block depth S36
High PONV risk (Apfel ≥3) Three to four prophylactic interventions under any approach S7; opioid sparing and TIVA reduce baseline risk S8
OSA (STOP-Bang 5–8 high risk S33) Minimize sedatives and opioids under any approach; enhanced monitoring per policy
Long-term opioid therapy Block + multimodal care is reasonable; continue the baseline regimen; do not abruptly discontinue S21
Opioid use disorder Continue the medication for OUD; do not withhold indicated opioids
Contraindications to the opioid-sparing adjuncts, or patient preference Approach 3 with opioid stewardship and monitoring
