# Measuring outcomes after TKA

> **Educational work by a pre-medical student — not medical advice.** Not a clinical protocol, prescription, or patient handout, and not for patient care. See [DISCLAIMER](DISCLAIMER.md).

Independent educational note, revised 27 October 2026. The former numerical MCID, responsiveness and universal recovery-timeline claims are withdrawn pending source-level reconstruction.

## Match the measure to the question

Pain intensity, physical function, quality of life, satisfaction and medication exposure are distinct outcomes. An improvement in one should not be described as improvement in all of them. Group-average change also does not establish that every patient experienced an important improvement.

[Roos et al., KOOS development, 1998, PMID 9699158](https://pubmed.ncbi.nlm.nih.gov/9699158/) describes a five-domain knee outcome instrument. The original study involved ACL reconstruction; it must not be described as original validation in a TKA cohort. Instrument version, language, population and follow-up matter.

[Terwee et al., 2021, PMID 34247326](https://pubmed.ncbi.nlm.nih.gov/34247326/) clarifies minimal important change and reviews PROMIS estimates. This is methodological evidence, not a universal knee-arthroplasty cutoff. No single MCID is assigned here.

## Reading questions

- What construct, scoring direction and scale version were used?
- Was the threshold intended for within-person change, between-group difference, or another question?
- Which anchor and population produced it, and how uncertain is the estimate?
- Are missing responses, floor/ceiling effects and measurement error reported?
- Does a robot-assisted comparison use a validated instrument without assuming identical recovery trajectories?

Questionnaire items, scoring forms and translations are not reproduced. Instrument-specific permissions and terms must be checked before any redistribution. This note does not provide a patient prognosis, consent script or treatment threshold.
